My Life My Health Chronic Condition Self Management Education

Senior fitness class.
Most older adults have at least one chronic condition such as arthritis, diabetes, lung or heart disease that may impact their ability to lead more active lives. My Life My Health, developed by Stanford University, is a highly interactive workshop that has been proven to help older adults better manage their chronic conditions, improve quality of life, and lower healthcare costs. This workshop meets for 2.5 hours for 6 consecutive weeks and is led by 2 leaders who have chronic conditions themselves. Registration is required.

Locker rooms and showers are available.
Childcare for children 6 and under is available in Cornerstone’s playroom and is included in the price
Additional members on household account should reside in household. Friends and relatives outside of your household must create a separate account for registration.
